I/We hereby request, instruct and authorise Selborne Foundation Trust to draw against my/our bank account, the amount necessary for the payment due in respect of a donation as set out in the online debit order detail document.  The withdrawal date is on the day of each month(or whatever frequency has been selected).

I understand that this amount will be withdrawn until 1 (one) calendar month’s written notification is provided.
All such withdrawals from my/our bank account by shall be treated as though they have been signed by me/us personally
